Who are our clients?
Eligible participants/clients are referred to Erhap House Care by their doctors, social and human services agencies, as well as friends and family members. Our professional staff will help navigate potential clients through the application process, which consists of:
- An initial evaluation
- A customized and personalized care plan addressing various areas of needed assistance (what type of assistance is needed and how often this assistance is required)
- Implementation of the personal care plan and working closely with the client and his/her physician to make sure clients’ needs are met daily.
