Erhap House Care GAFC program provides medical and psycho-social oversight, as well as Personal Care Attendants (PCA) to clients in qualified home settings. PCA services consist of aiding clients with their Activities of Daily Living (ADLs) – bathing, dressing, grooming, feeding, toileting, transferring, and ambulation. Care is provided seven days a week (based on an average of two {2} hours of personal caregiver staff per member per day).

Our GAFC program is supervised by the program director and the Multidisciplinary Care Team (MCT). The MCT develops and implements the Care Plan that is reviewed with the client before implementation.

GAFC is a Personal Care Assistance Program sponsored and paid for by MassHealth (Massachusetts Medicaid). This program keeps individuals safe at home and reduces the need for hospitalizations and admissions to nursing homes for more care. With the support of an adult companion in Massachusetts, individuals can maintain their independence while receiving necessary assistance. There is no out-of-pocket expense, and MassHealth pays for all the program’s costs. MassHealth allows residents in group homes or assisted living to also take part in GAFC.
